Registration To Care
Inquiry for late registrations are welcome! In addition to open availability, we can help you make a smooth transition. All you will need to do to transfer your care – is to make an appointment with us. Next, we will need a copy of your medical records from your previous provider. This is simply done by completing a medical records release form, that we have available for you in our practice. Lastly, your consultation itself does not guarantee registration, however after careful review -that you and baby are currently in good health then yes. We will be happy to welcome you!
